Group Home


We accept adults with serious mental illness who need 24-hour support and are stable enough for community living. Admission requires a clinical assessment, referral, and funding authorization—typically through Medicaid or private pay. We work closely with discharge planners to ensure a smooth transition. To be admitted, an individual must:
Age & Diagnosis
• Male resident, 18 years or older
• Have a primary diagnosis of serious mental illness (SMI) such as schizophrenia, bipolar disorder, major depression, or other qualifying conditions
Functional Need
• Require 24-hour supervision and support due to psychiatric symptoms, impaired functioning, or inability to live independently
• Show need for skill-building in daily living, medication management, and behavioral stabilization Assessment & Referral
• Have a comprehensive clinical assessment (often by a licensed mental health professional or hospital discharge planner)
• Be referred by a case manager, support coordinator, or hospital unit, with documentation of need for residential services, or by the individual themselves (certain qualifying circumstances)
Funding & Authorization: Be eligible for Medicaid, private pay, or other funding sources
• If Medicaid-funded, services must be authorized through the appropriate waiver or behavioral health benefit
Consent & Readiness
• Provide informed consent (or have a legal guardian do so)
• Be psychiatrically stable enough for a community-based setting (not requiring inpatient care)
• Ambulatory (able to move independently without assistance
